The Act, Communicate and Track (ACT) Planner is a key resource for participants in the Words to Action program. It is the advocacy experts’ playbook designed to guide you through the process of self-advocacy and help you track your progress.

Use the Planner to:

Discover your strengths and how to use them.

Understand your rights and how to assert them effectively.

Clearly express your needs and goals.

Set achievable goals and track your progress.

Overcome challenges and reflect on your experiences.

How to Use Your Workbook:

Identify the Problem: What change or solution do you want?

Decide the Outcome: What do you hope will happen?

Find the Pathway for Action: Who can help, and how can you contact them?

Take Action: Track your steps and ensure follow-through.

Follow-Up: Keep in touch with the relevant people to make sure the right support is in place.

Review the Outcome: Did it work? Do you need to take additional steps?
